This time, the second procedure was especially remarkable:
Two Spigelian hernias were discovered, one on each side, which is an extremely rare finding. Additionally, a previous incisional hernia repair on the left side made the abdominal wall closure particularly complex.

Primary and secondary ventral hernias up to sizes W2 can be treated by techniques like MILOS, EMILOS, e-TEP.
Laparoscopic IPOM repair is still a valuable alternative in recurrent hernias after extraperitoneal repair or in patients prone to postoperative wound complications.

At this online webinar we will be broadcasting live from the Knappschaftsklinikum Püttlingen in Germany.
The focus will be on the presentation of surgical procedures as well as the discussion of possible pitfalls and useful tips.

Furthermore the unrivalled advantage when using elastic implants will be clearly pointed out.
At the end we would like to discuss our standardized procedure with the participants.

The verbal real-time interaction between the trainer and the participants offers the possibility, exchange experiences and ask questions directly and at any time.

1. IPOM after incisional hernia repair

Age: 50 years
Height: 175 cm
Weight: 90 kg

Planned Surgery on 15.05.2025: Laparoscopic IPOM
Diagnosis: Multiple incisional hernias after laparotomy

Brief Medical History:

  • About 2 years ago: Surgery for endometrial carcinoma with lymphadenectomy of 45 lymph nodes (2 affected), followed by chemotherapy
  • Tumor-free for approximately 1 year

Clinical Examination:
At least 2 painful, reducible incisional hernias located just below the navel and just above the symphysis

2. IPOM Spegelian hernia

Age: 81 years
Height: 155 cm
Weight: 72 kg

BMI 30

Planned Surgery on 15.05.2025: Laparoscopic IPOM
Previous Surgery in 05/2024: SA sublay mesh repair
Diagnosis: Spigelian hernia, larger on the left than on the right

Brief Medical History:

  • History of hysterectomy (2014) via Pfannenstiel incision
  • History of laparoscopic sigmoid resection due to iatrogenic sigmoid perforation after colonoscopy (2021)
  • History of sublay surgery for suprapubic incisional hernia

DynaMesh®-IPOM implants are designed for soft tissue reinforcement and soft tissue bridging of the fascial and connective tissue structures of the abdominal wall as part of surgical treatment for epigastric hernias, umbilical or incisional hernias, and parastomal hernias following ostomy surgery.
